Patient selection: pulmonary sarcoidosis
Criteria for refractory disease:
(1) progressive lung disease despite >= 10 mg of prednisolone equivalent for >= 3 months
(2) need for additional disease-modifying anti-sarcoid drugs due to lack of efficacy, adverse effects or drug intolerance
(3) significant impairment (disability) as a result of progressive pulmonary disease
(4) exclusion of treatable complications (such as infection, cardiac disease or pulmonary hypertension) or poor compliance

• Additional drugs may include: methotrexate, azathioprine, leflunomide, infliximb, adalimumab, pirfenidone, nintedanib
