Description

Dispenzieri et al reported criteria for a hematologic response following therapy for light chain amyloidosis. The authors are from the Mayo Clinic.


Parameters:

(1) serum free kappa light chain in mg/dL prior to therapy

(2) serum free kappa light chain in mg/dL after therapy

(3) serum free lambda light chain in mg/dL prior to therapy

(4) serum free lambda light chain in mg/dL after therapy

(5) serum immunofixation electrophoresis after therapy

(6) urine immunofixation electrophoresis after therapy

 

difference between involved and uninvolved light chain =

= (involved light chain) - (uninvolved light chain)

 

decrease in percent =

= ((pretreatment) - (post-treatment)) / (pretreatment * 100%

 

To measure a decrease the light chain concentration should be >= 50 mg/dL.

 

Criteria for complete response:

(1) negative serum immunofixation electrophoresis

(2) negative urine immunofixation electrophoresis

(3) normal serum kappa to lambda ratio (from 3:2 to 2:1)
