Description

Davila et al reported criteria for the diagnosis of severe cytokine release syndrome (CRS) associated with CAR T-cell therapy. This can help to identify a patient at need for an urgent intervention. The authors are from Memorial Sloan-Kettering Hospital.


Patient selection: cytokine release syndrome

 

Criteria for severe cytokine release syndrome - all of the following:

(1) fever (temperature >= 38°C) for >= 3 consecutive days

(2) one or both of the following:

(2a) >= 1 serum cytokine concentration >= 250 times baseline

(2b) >= 2 serum cytokines with concentration >= 75 times baseline

(3) one or more of the following:

(3a) hypotension requiring >= 1 intraveneous pressor

(3b) oxygen saturation < 90% (hypoxia)

(3c) neurologic disorder (altered mental status, obtundation, seizures)
