Description

Cosio et al reported criteria for the diagnosis of an overlap syndrome between COPD and asthma (ACOS). The authors are from multiple institutions in Spain and participants of the CHAIN study.


Patient selection: COPD (age > 40 years, postbronchodilator FEV1 to FVC ratio < 0.7, and exposure to cigarettes)

 

Major criteria:

(1) previous history of asthma

(2) bronchodilator response to salbutamol (albuterol) (> 400 mL and > 15%)

 

where:

• The pulmonary function test measured for the bronchodilator response is not stated but is assumed to be FEV1 and percent of predicted.

 

Minor criteria:

(1) (serum IgE > 100 IU/mL) OR (history of atopy)

(2) percent eosinophils in the peripheral blood > 5%

(3) 2 separate bronchodilator responses to salbutamol (albuterol) (> 200 mL and > 12%)

 

The overlap syndrome is present if both are present:

(1) >= 1 major criterion

(2) >= 2 minor criteria

• The major and minor bronchodilator criteria can be present in the same patient.
