Description

Buffet et al used a number of criteria to describe the outcome following therapy for a papillary thyroid carcinoma. The authors are from the University of Paris VI.


 

Patient selection: papillary thyroid carcinoma

 

Criteria for progressive disease - 1 or both of the following:

(1) significant increase in serum thyroglobulin concentration

(2) imaging studies (CT or PET scan) abnormal

 

Another criteria for progression would be tumor identified in a pathology specimen from a new site.

 

Criteria for cure:

(1) serum thyroglobulin levels

(1a) < 2 ng/mL after withdrawal of L-thyroxine OR

(1b) undetectable on L-thyroxine OR

(1c) < 1 ng/mL after use of recombinant human TSH

(2) no iodine uptake after diagnostic whole body scan (WBS)

(3) normal neck ultrasonography

 

Criteria for stable disease:

(1) The patient does not meet criteria for cure or progression.

(2) The serum thyroglobulin concentration has been stable.

(3) The tumor volume is stable on imaging studies.
