Description

An international pediatric sepsis consensus conference reported criteria for the systemic inflammatory response syndrome (SIRS).

Parameters:

(1) core temperature (rectal, bladder, oral, central catheter) < 36°C or > 38.5°C

(2) abnormal heart rate in the absence of external stimulus, chronic drugs, etc - one of the following:

(2a) tachycardia: (mean heart rate > 2 SD above the normal for age) OR (unexplained persistent elevation over a 0.5 hour time period)

(2b) bradycardia: (mean heart rate < 10th percentile AND age < 1 year) OR (unexplained persistent depression over a 0.5 hour time period)

(3) abnormal respirations - one of the following

(3a) mean respiratory rate > 2 SD above normal for age

(3b) mechanical ventilation for an acute process not related to underlying neuromuscular disease or receipt of general anesthesia

(4) abnormal leukocyte count with one of the following:

(4a) elevated for age

(4b) decreased for age, not due to chemotherapy-induced leukopenia

(4c) > 10% immature neutrophils

Criteria for SIRS - both of the following:

(1) number of parameters present >= 2

(2) temperature and/or leukocyte count abnormal
