Description

The response to therapy in patients with chronic myelogenous leukemia (CML) in the chronic or accelerated phase can be evaluated based on changes in the hematologic parameters.

Complete hematologic response, or remission (CHR):

(1) normalization for at least 4 weeks

(2) bone marrow with < 5% blasts

(3) peripheral blood leukocyte count < 10,000 per µL without peripheral blasts, promeylocytes or myelocytes

(4) disappearance of all signs and symptoms related to CML

where:

• I assume that disappearance of all signs and symptoms include the platelet count <= 450,000 per µL and there is no clinical splenomegaly.

Partial hematologic response (PHR) - as for complete response except for:

(1) persistence of immature cells (myelocytes, metamyelocytes) in peripheral blood

(2) splenomegaly < 50% of the pretreatment level

(3) thrombocytosis > 450,000 per µL but < 50% of pretreatment level

Cytogenetic Response

Findings

complete

0% Philadelphia chromosome positive

partial

1-34%

minimal

35-90%

none

100% (> 90%)

A partial or complete cytogenetic response was classified as a major cytogenetic response.
