Sarcoidosis limited to the heart can be a challenge to diagnose.
Prerequisites:
(1) no evidence of sarcoidosis in any organ other than the heart
(2) 67Ga scintigraphy or 18F-FDG PET reveals no abnormal tracer uptake in any organ other than the heart
Major criteria:
(1) high-grade AV block or fatal ventricular arrythmias
(2) basal thinning of the ventricular septum OR abnormal ventricular wall anatomy
(3) left ventricular contractile dysfunction or focal ventricular wall asynergy
(4) 67Ga citrate scintigraphy or 18F-FDG PET with high tracer uptake in the heart
(5) gadolinium-enhanced MRI reveals delayed contrast enhancement of the myocardium
Isolated cardiac sarcoidosis is diagnosed when:
(1) major criterion 4 is present
(2) 3 or 4 of the other major criteria are present
