Intrapartum antibiotic prophylaxis (IAP) may or may not be adequate to prevent neonatal infection with Group B Streptococcus.
Patient: pregnant woman in the intrapartum period prior to delivery who is a candidate for prophylaxis due to risk factors for neonatal sepsis
Criteria for adequate IAP - all of the following:
(1) therapy with penicillin, ampicillin or cefazolin
(2) correct dose administered
(3) correct route of administration
(4) given for >= 4 hours
Therapy with clindamycin or vancomycin may be necessary if the mother is allergic to beta-lactams but may not be adequate to prevent neonatal infection even when given properly.
