Description

Traube et al reported the Cornell Assessment of Pediatric Delirium (CAPD) to describe delirium in a pediatric patient in the PICU. The authors are from Weill Cornell Medical College, Memorial Sloan-Kettering Cancer Center, NY Presbyterian Hospital and Jefferson Medical College.


Patient selection: <= 21 years

 

Exclusion: Richmond Agitation and Sedation Scale -4 or -5

 

Items:

(1) child makes eye contact with caregiver

(2) child's actions purposeful

(3) child aware of surroundings

(4) child communicates needs and wants

(5) child restless

(6) child inconsolable

(7) child underactive (very little movement while awake)

(8) child takes a long time to respond to interactions

Interpretation:

• minimum score: 0

• maximum score: 32

• A cutoff of >= 9 had a sensitivity of 94% and specificity of 79%.

• The score is sensitive (96%) but shows lower specificity (51%) in developmentally delayed patients.

 

Performance:

• The area under the ROC curve us 0.86 in children with developmental delay.

• The negative predictive value is 0.985.
