Description

Administration of aminoglycosides with dosing at extended intervals (once a day, single daily dose) can be effective and economical, However, the technique should not be used when certain conditions are present.


 

Contraindications to the use of

extended interval aminoglycoside dosing

 

(1) hypersensitivity to aminoglycosides (avoid aminoglycosides entirely)

 

(2) infants and children <= 12 years old

(3) severely debilitated, elderly patients

 

(4) pregnancy

(5) postpartum state (within 2 months of delivery)

(6) breastfeeding

 

(7) impaired renal function (creatinine clearance <= 20 mL/min)

(8) dialysis

 

(9) severe burns > 20% body surface area (rapid excretion)

(10) cystic fibrosis (rapid excretion)

 

(11) Enterococcal endocarditis

(12) Pseudomonas aeruginosa infection (resistance)

 

(13) ascites

(14) cirrhosis

 

(15) neutropenia (absolute neutrophil count < 1,000 per µL)

 

(16) significant obesity

 

(17) patients with a history of hearing loss
