Description

A patient with severe Chagas Cardiomyopathy may not be a suitable candidate for heart transplant if certain conditions are present.


Patient: severe Chagas cardiomyopathy

 

Absolute contraindications for cardiac transplant:

(1) fixed pulmonary vascular resistance > 5 Woods units, even after pharmacological tests

(2) megacolon or megaesophagus severe enough to jeopardize the results of the transplant

(3) advanced liver failure

(4) severe pulmonary disease

(5) ABO incompatibility in prospective cross-match tests between recipient and donor

(6) severe psychiatric disease

(7) chemical dependence

(8) non-adherence to medical recommendations

 

Relative contraindications to cardiac transplant:

(1) unfavorable psychological condition

(2) comorbidities impairing long-term life expectancy

(3) morbid obesity

(4) active systemic infection

(5) active peptic ulcer

(6) pulmonary embolism for less than 3 weeks

(7) neoplasm (unless oncologist gives clearance)

(8) difficult-to-control diabetes

(9) renal failure with creatinine clearance less than 30 mL per min per 1.73 sw m

(10) unfavorable social conditions

(11) lymphocyte panel > 10% (not defined in paper)
