Description

Hunter et al evaluated immunocompetent patients following an acute diarrheal episode caused by cryptosporidiosis. Some patients develop one or more complaints that could be reactive. The authors are from the University of East Anglia, the University of Central Lancashire, and multiple hospitals in England.

Species of cryptosporidium involved:

(1) C. hominis (C. parvum genotype 1): specialized for humans

(2) C. parvum (C. parvum genotype 2): infect several species including humans

Sequelae:

(1) relapse of gastrointestinal symptoms (vomiting, abdominal pain, diarrhea, decreased appetite) was seen in 41% of patients

(2) one ore more reactive symptoms were seen in almost half the patients infected with C. hominis:

(2a) joint complaints (arthropathy, with pain, stiffness, and/or swelling)

(2b) eye pain

(2c) recurrent headaches

(2d) dizzy spells

(2e) fatigue

These sequelae may be moderate to severe in severity, causing prolonged disability in affected patients. Patients may benefit from continued followup after recovery from the initial diarrheal disease.
