A number of conditions may cause changes in the electrocardiogram that are the same as those seen with Brugada syndrome.
Conditions that may phenocopy Brugada syndrome:
(1) hypokalemia or hyperkalemia
(2) other metabolic disorders (hypothyroidism, hyponatremia, hypercalcemia)
(3) coronary ischemia
(4) pericardial disease
(5) extracardiac mechanical compression (pectus excavatum, anterior mediastinal mass, mediastinal tumor)
(6) pulmonary embolism
Conditions that may unmask Brugada syndrome and not phenocopies:
(1) fever
(2) sodium channel blocker
(3) other drugs (acetylcholine, edrophonium, calcium blockers, nitrates, alcohol, cannabis, potassium channel openers, ergonovine, alpha-adrenergics)
