Endocrine disorders:
(1) thyrotoxicosis
(2) glucagonoma
(3) ? pheochromocytoma
Supraventricular tachyarrhythmia:
(1) atrial fibrillation
(2) atrial flutter
(3) atrial tachycardia
(4) AVRT (AV reciprocating tachycardia)
(5) AVNRT (AV nodal re-entrant tachycardia)
Permanent form of junctional tachyarrhythmia:
(1) persistent junctional reciprocating tachycardia (PJRT)
(2) atrial pacing at high rates
Ventricular tachyarrhythmia:
(1) premature ventricular complexes (PVCs)
(2) right ventricular outflow tract ventricular tachycardia
(3) idiopathic left ventricular tachycardia
(4) bundle-branch re-entry ventricular tachycardia
(5) myocarditis
(6) ventricular pacing at high rates
