The Comprehensive Frailty Severity Index (FSI) for End-Stage Liver Disease can be used to identify frailty in a patient undergoing liver transplant. This consists of multiple components, including physical performance and laboratory measures. The authors are from the University of Miami.
Patient selection: liver transplant recipient
The total score is a sum of:
(1) physical performance
(2) laboratory markers
(3) nutritional assessment
Interpretation:
• minimum score: 1
• maximum score: 35
• A score >= 14 indicates frailty. It is also associated with a worse 1-year survival.
Other outcomes associated with an elevated FSI:
(1) length of ICU stay
(2) length of hospital stay
(3) duration of mechanical ventilation
(4) major complications
Predictors of an FSI >= 14:
(1) male sex (OR 2.9)
(2) pretransplant hospitalization (OR 2.5)
(3) MELD score (OR 1.08 per point)
