Description

Typhoid fever is a systemic infection which may have a number of complications, some of which may leave to death or long-term disability. A patient with typhoid fever should be monitored for these complications.


 

Organism: Salmonella enterica serotype typhi

 

Complications:

(1) hepatitis with elevated liver function tests

(2) cholecystitis

(3) myocarditis

(4) meningitis

(5) confusion, agitation, delirium or psychosis

(6) bronchitis or pneumonia

(7) pharyngitis

(8) miscarriage or neonatal typhoid (if pregnant female)

(9) abscess

(10) DIC (usually subclinical and rarely clinically significant)

(11) seizure activity in small children under 5 years of age

 

Organ failures associated with severe disease:

(1) gastrointestinal perforation with peritonitis

(2) gastrointestinal hemorrhage, which may be massive

(3) shock

(4) encephalopathy

 

Other problems:

(1) relapse

(2) reinfection

(3) chronic carrier state ("Typhoid Mary", which may be more common in patients with cholelithiasis or schistosomiasis)
