Iliac arterial embolization in a patient with pelvic trauma may be complicated by a range of problems. The authors are from University of Cincinnati, University of Texas Southwestern, University of Virginia and University of Louisville.
Patient selection: pelvic trauma with arterial hemorrhage
Procedure: iliac arterial embolization
Complications may include:
(1) gluteal muscle necrosis
(2) rectal necrosis
(3) neurologic deficits
(4) gait change
(5) sexual dysfunction
(6) skin necrosis (which may be confused with a decubitus ulcer)
The earlier that a complication is identified the better.
Reasons for a delay in diagnosis:
(1) the patient is unconscious or sedated
(2) the patient is not monitored closely
(3) the patient has a sensory deficit to the area affected
