Description

Complement Activation-Related Pseudoallergy (CARPA) is a reaction that explains some cases of anaphylactoid reactions seen during infusion of certain drugs.


Clinical features:

(1) exposure to a triggering drug

(2) activation of complement, with reduction in C3 and C4 plasma concentrations and appearance of split products

(3) pseudoallergic or hypersensitivity reaction, which can be severe

(4) exclusion of other explanations

 

Triggers may include:

(1) liposomal or micellar agents

(2) enzymes

(3) antibodies, including anti-thymocyte globulin

(4) superparamagnetic iron oxide nanoparticles (SPIONs)

(5) radiocontrast media
