Description

Yoo et al used a combination of the modified early warning score (MEWS) and blood lactate to identify an inpatient with sepsis to transfer to the intensive care unit (ICU). The authors are from Asan Medical Center and the University of Ulsan in Seoul.


Patient selection: age >= 18 years, sepsis, on general ward for > 24 hours, systolic blood pressure < 90 mm Hg

 

Criteria for ICU transfer - one or more of the following:

(1) sustained shock requiring vasopressors

(2) acute respiratory failure

(3) acute heart failure or pulmonary edema

(4) acute renal failure requiring hemodialysis

(5) drowsy (altered mental status)

 

Cut-off values for ICU transfer:

(1) MEWS >= 5.5 (MEWS range 0 to 14; a score >= 5 is usual trigger; since the score is an integer the cut-off needs to be 5 or 6))

(2) blood lactate concentration >= 3.05 mmol/L

 

Performance:

The area under the ROC curve is 0.9.
