von Arnim et al reported a clinical symptom tool for the diagnosis of eosinophilic esophagitis in adults. The authors are from Otto von Guericke University in Magdeburg, Germany.
Patient selection: adult with esophageal symptoms
Clinical findings:
(1) history of atopy
(2) history of food impaction
(3) symptoms refractory to proton pump inhibitor therapy
(4) elevated Ig E (> 100 U/L) and/or eosinophilia (absolute > 500 per µL) in the blood
Interpretation:
• minimum number of findings: 0
• maximum number of findings: 4
• Eosinophilic esophagitis should be considered if the number of findings is >=2.
Performance:
• The area under the ROC curve is 0.95 (although sensitivity is 91.3% and specificity 86.3%).
