Shibahashi et al proposed a rule for determining when emergency responders should terminate resuscitation of a cardiac arrest patient. The authors are from Tokyo Metropolitan Bokutoh Hospital in Japan.
Patient selection: out-of-hospital cardiac arrest
Outcome: death or unfavorable neurological status at 1 month after arrest
Termination of resuscitation rule - all of the following:
(1) unshockable initial rhythm (adjusted odds ratio 6.1)
(2) unwitnessed by bystander s(adjusted odds ratio 5.3)
(3) age >= 73 years (adjusted odds ratio 2.3)
This rule does not incorporate how the patient responds to resuscitation attempts.
Performance for unfavorable neurological outcomes:
• The area under the ROC curved 0.83. The specificity was 0.95.
Performance for death:
• The area under the ROC curved 0.79. The specificity was 0.92.
