Description

Sasaki et al reported a simple score for identifying an outpatient receiving maintenance hemodialysis who may have bacteremia. This can help to identify a patient who may benefit from more aggressive management. The authors are from multiple institutions in Japan.

BAC-HD = body temperature, alkaline phosphatase, CRP, heart rate, drug)

Patient selection: outpatient maintenance hemodialysis

Outcome: bacteremia (present in 16% of study cohort)

Parameters:

(1) body temperature in °C

(2) heart rate in beats per minute

(3) serum CRP in mg/dL

(4) serum alkaline phosphatase in IU/L

(5) prior antibiotics within 1 week

= SUM(points for all 5 parameters)

Interpretation:

minimum score: 0

maximum score: 5

A score >= 2 is associated with an increased risk of bacteremia.

Performance:

A score >= 2 has a sensitivity of 90% and specificity of 51%.

The area under the ROC curve is 0.76.
