Description

Newgard et al reported a clinical prediction rule for interhospital transfer of a patient with blunt traumatic brain injury (TBI). The rule can help to identify a patient who is at significant risk for requiring high therapeutic intensity. The authors are from Oregon Health & Science University.

Patient selection: 18-65 years of age AND serious blunt head injury

High therapeutic intensity may include any of the following within 72 hours:

(1) neurosurgery

(2) exploratory laparotomy

(3) vasopressor support

(4) >= 6 units packed RBCs

(5) death

Parameters:

(1) Glasgow coma scale (GCS) in the ED

(2) intubation

(3) respiratory rate in the ED

(4) age in years

(5) temperature in °C in the ED

(6) heart rate in the ED

Parameter (in sequence)

Risk

GCS < 13 OR intubated

high

respiratory rate < 16 beats per minute

high

age 32 to 65 years of age

moderate

temperature< 36.6°C

high

pulse > 98 beats per minute

high

else

low
