Peeters et al evaluated stomal function in patients treated for rectal cancer using a simple Likert scale. This can be monitored over time to identify problems and to determine the effectiveness of interventions. The authors are from the Dutch Colorectal Cancer Group.
Parameters monitored:
(1) skin irritation around the stoma
(2) smell
(3) bleeding from the stoma
(4) leakage around the stoma (stool, blood, mucus)
(5) pain at the stoma
(6) noise

This is scale does not indicate how severe a problem is. Other instruments have included a combination score based on frequency and severity levels. for each finding.
To keep the score simple, a modified scoring scheme might be based on how much of a problem the finding has been for the patient. This is point assignment is not linear since the impact on the patient is not linear.
