Description

Nanophyetiasis is a small intestinal trematode that can cause human infection after handling or eating raw or improperly cooked salmonid fish.


 

Synonym: salmon poisoning disease (SPD) in dogs

 

Species: Nanophyetus salmincola

 

Distribution: Siberia and Pacific Northwest

 

Fishes that may serve as source:

(1) coho salmon (Onchorhynchus kisutch)

(2) steelhead trout

 

Exposure may follow ingestion of metacercariae after:

(1) handling highly infected fish (probably eating with unwashed hands)

(2) eating raw fish

(3) eating incompletely cooked fish

(4) eating home-smoked fish

(5) eating kippered (salted and smoked) fish

(6) eating fish eggs

 

Symptoms in humans:

(1) chronic diarrhea

(2) nausea and vomiting

(3) abdominal discomfort

(4) weight loss

(5) fatigue

(6) gas with flatus

(7) bloating

 

Some patients may be clinically asymptomatic.

 

Laboratory findings:

(1) eosinophilia in the peripheral blood

(2) bipolar operculated eggs identified on stool examination for ova and parasites (64 to 97 microns long, 34 to 55 microns wide)
