Description

The presence of certain findings should prompt an evaluation for cow's milk allergy in an infant.


 

Clinical and laboratory findings that may indicate the presence of an allergy to cow's milk:

(1) temporal relationship between symptoms and ingestion of cow's milk

(2) family history of atopy and/or cow milk allergy in siblings

(3) one or more of the following:

(3a) atopic dermatitis

(3b) asthma

(3c) infantile colic

(3d) gastroesophageal reflux (GERD)

(3e) diarrhea with proctitis (often with rectal bleeding)

(3f) diarrhea with malabsorption

(3g) enterocolitis

(3h) failure to thrive

(3i) eosinophilic gastroenteropathy

(3j) anaphylaxis

(3k) anemia

(3l) eosinophilia

(4) exclusion of other conditions, especially lactose intolerance

 

The workup to confirm the diagnosis may include:

(1) oral challenge testing and/or dietary exclusion

(2) skin prick test

(3) RAST testing
