Description

Loro Morejon et al evaluated patients coinfected with HIV and paracoccidioidomycosis in Brazil. The clinical features in this group differ from those seen in the classic infection affecting HIV-negative adult males. The authors are from the University of Sao Paulo in Brazil.

Clinical features of paracoccidioidomycosis in HIV-infected patients:

(1) younger age (24 to 44 years of age)

(2) fever

(3) lymphadenopathy

(4) hepatosplenomegaly

(5) multiple skin lesions

(6) ulcerations of the oral and nasal mucosa

(7) chronic hoarseness.(suggesting infection of the larynx)

Most patients (84%) had a CD4-positive lymphocyte count < 200 per µL. Thus paracoccidioidomycosis should be considered as an AIDS-defining opportunistic infection in this population.
