Description

Osteochondritis dissecans occurs when there is repetitive stress across the radiocapitellum joint.


 

Location: capitellum of the humerus (located just medial to the lateral epicondyle)

 

Occurrence:

(1) gymnastics

(2) throwing sports

(3) any sport that increases the load across the elbow

 

Clinical features:

(1) The injury usually occurs in adolescents and involves the dominant arm.

(2) There is a gradual onset of pain in the lateral elbow.

(3) The pain is aching and dull that is made worse by motion.

(4) Locking or catching of the elbow joint may occur.

(5) There is tenderness to palpation at the radiocapitellum joint.

(6) A joint effusion may be present.

(7) Palpable crepitus may be present in the radiocapitellum joint.

(8) Some limitation in elbow extension is common.

(9) An X-ray at the elbow may show a crescent-shaped region of sclerotic subchondral bone with a lucent rim, with or without a loose body.
