Description

Pediatric patients may occasionally develop the juvenile pattern of paracoccidioidomycosis. Patients who are severely immunocompromised may follow a similar course.

Clinical features of the juvenile form:

(1) pediatric age group (most cases of paracoccidioidomycosis occur in adult males over 30 years of age)

(2) acute or subacute disseminated disease with fever and weight loss (versus the usual chronic course)

(3) generalized lymphadenopathy

(4) hepatosplenomegaly with or without jaundice

(5) expanded mediastinum (from lymph node involvement)

(6) intra-abdominal masses

(7) ascites

(8) lesions of bones and/or joints

(9) variable cutaneous lesions

Lesions of the lungs and mucous membranes are uncommon.

The infection may be progressive. Severe infections have a poor prognosis.
