Description

Ejaculatory duct obstruction (EDO) may be an explanation for male infertility. The ejaculatory duct is formed by the confluence of the seminal vesicle and terminal ampullary portion of the vas deferens.


 

Features of sperm sample of an affected male:

(1) low volume ejaculate

(2) low seminal pH

(3) marked oligozoospermia or azoospermia

 

Transrectal ultrasonography:

(1) dilated seminal vesicle with diameter > 1.5 cm

(2) midline prostatic or retroprostatic cysts (may be causing the duct obstruction)

(3) ejaculatory duct cyst

(4) calcifications in ejaculatory duct

(5) ejaculatory duct dilatation

 

The obstructed lumen of the ejaculatory duct may be seen on sagittal ultrasound images as a hypoechoic tubular structure entering the urethra at the level of the verumontanum.

 

Features of fluid aspirated from a dilated seminal vesicle:

(1) high concentration of mobile sperm

(2) normal spermatozoa morphology
