Description

Infections by the Bourbon virus have only recently been identified and are being actively investigated.


 

Site of initial case: Bourbon County, Kansas

 

Virus: genus Thogotovirus (family Orthomyxoviridae)

 

Transmission: presumed tickborne

 

Clinical features:

(1) fever

(2) nausea

(3) diarrhea

(2) anorexia

(4) myalgia and arthralgia

(5) headache

(6) chills

(7) absence of respiratory symptoms initially

(8) altered mental status

(9) diffuse maculopapular rash

(10) lymphadenopathy

(11) multi-organ failure

(12) failure to respond to doxycycline

 

Laboratory features:

(1) leucopenia

(2) thrombocytopenia

(3) elevated liver function tests

 

Diagnosis involves exclusion of other causes.

 

A confirmed case should have evidence of virus RNA by RT-PCR or rise in antibody titer.
