Description

Chilblain (blain = blister) Lupus Erythematosus Hutchinson is a form of cutaneous lupus erythematosus that occurs on cold-exposed skin surfaces. Some patients may develop systemic lupus erythematosus.


 

Sites of involvement:

(1) fingers and toes

(2) soles and heels of the feet

(3) palms of the hands

(4) tip of the nose

(5) ears

(6) elbows

(7) knees

 

Lesions may include:

(1) papules

(2) plaques

(3) blisters

(4) scarring with atrophic plaques

(5) telangiectasia

(6) hyperkeratotic lesions

(7) fissures

(8) ulcers

 

Lesions may range in color from erythematous to violaceous.

 

Clinical features may include:

(1) familial inheritance (may be associated with mutation in TREX1)

(2) can be painful and cause disability

(3) tend to persist for weeks after cold exposure has stopped

 

Differential diagnosis:

(1) cold agglutinin disease, cryoglobulinemia

(2) frostbite

(3) cutaneous sarcoidosis

(4) vasculitis

(5) Raynaud's phenomenon

(6) CMML
