Description

Paek et al reported a clinical decision rule for identifying a pediatric patient who may have a fracture of the orbital wall. This can help to guide additional imaging studies. The authors are from Seoul National University, SMG-SNU Boramae Medical Center, and Kangwon National University in Seoul.

Patient selection: pediatric patient with head trauma

Parameters:

(1) orbital rim tenderness

(2) periorbital ecchymosis

(3) painful extraocular movement

(4) nausea and vomiting

= SUM(points for all 4 parameters)

Interpretation:

minimum score: 0

maximum score: 4

A score >= 1 is associated with an increased risk for having an orbital fracture.

Performance:

The area under the ROC curve was 0.79.
