Chordomas are rare tumors that present differently based upon location. One group occurs at the skull base along the cephalad end of the primitive notochord.
Patient selection: chordoma arising in the skull base
Clinical findings:
(1) abducens (cranial nerve 6) palsy
(2) palsies of lower cranial nerves
(3) headache
(4) metastasis
On skull X-rays there may be a lytic lesion of bone with periosteal elevation.
On MRI it:
(1) is hypointense on T1-weighted images
(2) is hyperintense on T2-weighted images
(3) shows contrast enhancement
