Description

Inadequate dialysis often goes undetected but may be manifested by a variety of clinical or laboratory findings. Patients receiving hemodialysis should have the delivered dose of dialysis measured regularly to ensure adequate therapy is being administered.


 

Clinical Findings:

(1) anorexia, vomiting or nausea

(2) peripheral neuropathy

(3) weakness or poor functional status

(4) impaired alertness

(5) pericarditis

(6) ascites

(7) very low weight gain between treatments

(8) unexplained, persistent weight loss

(9) fluid retention or hypertension

(10) diarrhea

 

Laboratory Findings:

(1) urea reduction ratio < 65%

(2) Kt/V ratio < 1.2

(3) low serum albumin concentration (< 4 g/dL)

(4) low predialysis blood urea nitrogen concentrations (< 50 mg/dL)

(5) low predialysis serum creatinine concentration (< 5 mg/dL)

(6) persistent anemia (hematocrit < 30%) despite epoetin therapy

 

where:

• urea reduction ratio = 1 - ((postdialysis BUN) / (predialysis BUN))

• low serum albumin, blood urea nitrogen and/or creatinine may be indications of malnutrition
