(2) body mass index (BMI) in kg per square meter
(3) ASA status
1 or 2
ASA 2 to 5
3 or 4
Factors making intubation difficult:
(1) insufficient mouth opening
(2) blood or secretions in the airway
(3) concomitant cervical spine injury
