Description

The AO-ASIF (Arbeitsgemeinschaft fur Osteosynthesefragen, Association for the Study of Internal Fixation) developed a classification system for fractures, based on location and type of fracture.

Code for location: 44 (tibia/fibula, malleolar segment)

Locations:

(1) infrasyndesmotic lesion (A)

(2) transsyndesmotic fibular fracture (B)

(3) suprasyndesmotic lesion (C)

Infrasyndesmotic Lesion

Type of Fracture

Fracture Code

infrasyndesmotic lesion, isolated

rupture of the lateral collateral ligament

A1.1

avulsion of the tip of the lateral malleolus

A1.2

transverse fracture of the lateral malleolus

A1.3

infrasyndesmotic lesion, with fracture of the medial malleolus

rupture of the lateral collateral ligament

A2.1

avulsion of the tip of the lateral malleolus

A2.2

transverse fracture of the lateral malleolus

A2.3

infrasyndesmotic lesion, with postero-medial fracture

rupture of the lateral collateral ligament

A3.1

avulsion of the tip of the lateral malleolus

A3.2

transverse fracture of the lateral malleolus

A3.3

Transsyndesmotic Fibular Fracture

Type of Fracture

Fracture Code

transsyndesmotic fibular fracture, isolated

simple

B1.1

simple, with rupture of the anterior syndesmosis

B1.2

multi-fragmentary

B1.3

transsyndesmotic fibular fracture, with medial lesion

simple, with rupture of the medial collateral ligament and rupture of the anterior syndesomosis

B2.1

simple, with fracture of the medial malleolus and with rupture of the anterior syndesmosis

B2.2

multi-fragmentary

B2.3

transsyndesmotic fibular fracture, with medial lesion and a Volkmann

fibula simple, with rupture of the medial collateral ligament

B3.1

fibula simple, with fracture of the medial malleolus

B3.2

fibular multi-fragmentary, with fracture of the medial malleolus

B3.3

where:

• A Volkmann is a fracture of the postero-lateral rim.

Suprasyndesmotic Lesion

Type of Fracture

Fracture Code

suprasyndesmotic lesion, diaphyseal fracture of the fibula, simple

with rupture of the medial collateral ligament

C1.1

with fracture of the medial malleolus

C1.2

with fracture of the medial malleolus and a Volkman (Dupuytren)

C1.3

suprasyndesmotic lesion, diaphyseal fracture of the fibula, multi-fragmentary

with rupture of the medial collateral ligament

C2.1

with fracture of the medial malleolus

C2.2

with fracture of the medial malleolus and a Volkman (Dupuytren)

C2.3
