Description

Siewert et al classified adenocarcinomas at the gastroesphageal junction into different types based on the topographic location. The type may influence the surgical procedure performed and the subsequent outcome. The authors are from the Technische Universitat Munchen in Munich, Germany.

Type

Location of Tumor Center

Classification

I

> 1 cm above the gastroesophageal junction

adenocarcinoma of the distal esophagus with downward extension

II

from 1 cm above to 2 cm below the gastroesophageal junction

true adenocarcinoma of the gastric cardia

III

> 2 cm below the gastroesophageal junction

subcardial gastric adenocarcinoma with upward extension

Type I adenocarcinoma typically arises from the specialized intestinal metaplasia seen in Barrett's esophagus. I modified the classification above to classify a tumor arising out of Barrett's esophagus with a tumor center > 0.5 cm above the GE junction as Type I.
