Description

Seinsheimer classified fractures of the distal femur using a scheme based on the pattern of involvement. The author is from the Massachusetts General Hospital.

Parameters:

(1) involvement of the lateral condyle

(2) involvement of the medial condyle

(3) involvement of the supracondylar distal metaphysis

(4) displacement

(5) comminution

IIA

supracondylar distal metaphysis

> 2 pieces or comminuted

IIA

fracture involves the intercondylar notch, medial condyle separates

IIIA

fracture involves the intercondylar notch, lateral condyle separates

IIIB

fracture involves the intercondylar notch, both condyles separate

IIIC

fracture involves the intercondylar notch, both condyles separate. plus transverse fracture in supracondylar distal metaphysis

IIIC

fracture involves the articular surface of the medial condyle, medial condyle separates

2 pieces or comminuted

IVA

fracture involves the articular surface of the lateral condyle, lateral condyle separates

2 pieces or comminuted

IVB

see below

multiple pieces or comminuted

IVC

Combinations involved in Type IVC fractures - any or all of the following:

(1) one or both articular surfaces for the condyles

(2) one or both condyles extending to the intercondylar notch

(3) supracondylar distal metaphysis
