Schenck developed a classification system for fractures and dislocations involving the proximal interphalangeal joint. The author is from Rush-Presbyterian-St. Luke's Medical Center in Chicago.
Parameters:
(1) grade of articular fracture involving the base of the middle phalanx
(2) grade of dislocation and/or subluxation
(3) fracture of the proximal phalangeal head
percent of articular surface for the middle phalangeal base involved by fracture |
Grade |
<= 10% |
I |
11 – 20% |
II |
21 – 40% |
III |
> 40% |
IV |
degree of dislocation/subluxation between the head of the proximal phalanx and the base of the middle phalanx |
Grade |
< 25% |
A |
25 – 50% |
B |
> 50% but subtotal |
C |
total |
D |
Fractures of the proximal phalangeal head may be:
(1) unicondylar or
(2) bicondylar
The fragments of the condylar fracture may be:
(1) nondisplaced (stay in place) or
(2) displaced (separated)
