Description

Patients with sickle cell disease may develop a proliferative retinopathy. Goldberg developed a classification scheme for evaluating and monitoring these patients. The author is from the Johns Hopkins Hospital and University of Illinois Eye and Ear Infirmary.


 

Examination: indirect ophthalmology and fluorescein angiography

 

Parameters (aka Stages):

(1) peripheral arteriolar occlusions (Stage I)

(2) peripheral arteriolar-venular anastomoses (Stage II)

(3) neovascular and fibrous proliferations, including "sea fan" formation (Stage III)

(4) vitreous hemorrhage (Stage IV)

(5) retinal detachment (Stage V)

 

Each parameter is evaluated based on the extent of circumferential involvement.

 

The first 3 parameters may also be staged using a quantitative count of findings.
