Primary angle closure (PAC) may be associated with a spectrum of findings. The authors are from the Institute of Ophthalmology in London.
Parameters:
(1) occludable drainage angle between peripheral iris and the posterior trabecular meshwork
(2) evidence of trabecular obstruction by the peripheral iris
(3) glaucomatous damage involving the optic disc
For epidemiological studies the occludable drainage angle may be defined as an angle in which >= 270° of the posterior trabecular network cannot be seen
Evidence of trabecular occlusions - one or more of the following:
(1) peripheral anterior synechiae
(2) elevated intraocular pressure
(3) iris whirling (distortion of the radially oriented iris fibers)
(4) glaucomfleken lens opacities
(5) excessive pigment deposition on the trabecular surface
