Arteriography can be used to identify arterial lesions in the penile blood supply. While rarely performed today it can still be useful for some patients. Benson et al developed a classification scheme for arterial lesions seen on arteriography.
Selective arteriography is performed on the left and then the right internal pudendal arteries following intracavernosal injection of 60 mg of papaverine. 10-15 mL of low osmolality contrast medium was used in the injection for each artery. Image subtraction may improve the quality of the images.
Arteries examined:
(1) left and right internal pudendal arteries
(2) left and right common penile arteries
(3) left and right cavernosal arteries
Findings on Arteriography
Classification
no abnormalities
normal
single area with > 50% stenosis
moderate
occlusion of any of the arteries
severe
multiple areas with > 50% stenosis the arteries on one side
severe
any findings not meeting the above categories
mild
