Description

Simpson et al monitored the change in splenic volume associated with chemotherapy in patients undergoing hepatic resection for metastatic colorectal cancer. Patients with a significant increase in splenic volume were at greater risk for complications following surgery. The authors are from Memorial Sloan Kettering Cancer Center and Vanderbilt University.


 

Patient selection: metastatic colorectal cancer with chemotherapy for 6 months prior to resection of metastases

 

Imaging studies: CT or MRI with splenic volumetry

 

Parameters:

(1) splenic volume at baseline in mL

(2) splenic volume 6 months later

 

change in splenic size =

= ((splenic volume at 6 months) - (splenic volume at baseline)) / (splenic volume at baseline) * 100%

 

Interpretation:

• An increase in splenic volume >= 39% was associated with major complications following surgery.
