Dextrocardia (with situs inversus) is associated with changes in the 12-lead electrocardiogram.
ECG changes
(1) right axis deviation
(2) negative QRS complex, inverted P, and inverted T waves in lead I
(3) biphasic T waves and progressively decreasing R-wave amplitude from V1 to V6 (precordial leads)
(4) upright P wave in AVL
Right chest repositioning of V1 to V6 shows normal R-wave progression in the precordial leads.
