Does the patient complain of persistent or relapsing fatigue or easy fatigability that does not resolve with bed rest?

Is the fatigue severe enough to reduce average daily activity by >= 50%?

Does a careful history & review of medical records show any evidence of pre-existing chronic medical conditions?

Does the patient have any previous history of significant mental illness?

Does the patient have a mild fever (37.5-38.6 °C oral, if documented by patient)?

Does the patient experience chills?

Does the patient have a sore throat?

Is there lymph node pain in the anterior cervical chain?

Is there lymph node pain in the posterior cervical chain?

Is there lymph node pain in the axillary chains?

Has the patient had unexplained, generalized muscle weakness?

Does the patient have muscle discomfort or myalgias?

Does the patient show prolonged ( >= 24 hours) generalized fatigue after previously tolerable levels of exercise (post-exertional weakness)?

Does the patient complain of new, generalized headaches?

Does the patient have migratory, noninflammatory arthralgia?

Does the patient have photophobia?

Does the patient complain of transient visual scotomata?

Has the patient complained of forgetfulness?

Does the patient show excessive irritability?

Does the patient complain of feeling confused?

Does the patient have difficulty thinking or inability to concentrate?

Does the patient show signs of depression?

Does the patient have disturbances in sleep?

Was the initial onset of symptoms acute or subacute?

Does the patient show a low-grade fever (37.6-38.6 °C oral, 37.8-38.8 °C rectal)?

Did the patient have low grade fever for more than a month?

Does the patient have a nonexudative pharyngitis?

Did the patient have nonexudative pharyngitis for more than a month?

Are anterior cervical lymph nodes palpable (but < 2 cm in diameter) or tender?

Are posterior cervical lymph nodes palpable (but < 2 cm in diameter) or tender?

Are axillary lymph nodes palpable (but < 2 cm in diameter) or tender?

Did the patient have palpable or tender lymph nodes more than a month ago?
