Description

The Centers for Disease Control and Prevention (CDC) has reported criteria for the diagnosis of Multisystem Inflammatory Syndrome in children (MIS-C) who are infected with COVID-19.


Criteria:

(1) age < 21 years old

(2) (fever >= 38°C or report of subjective fever) lasting >= 24 hours

(3) one or more positive laboratory tests

(4) clinically severe illness requiring hospitalization

(5) multi-system organ involvement with involvement of 2 or more of the following:

(5a) cardiac

(5b) renal

(5c) respiratory

(5d) hematologic

(5e) gastrointestinal

(5f) dermatologic

(5g) neurological

(6) no other diagnosis is identified that can explain the findings better

(7) one or more of the following:

(7a) positive RT-PCR for SARS-CoV-2

(7b) positive serology for antibodies against SARS-CoV-2

(7c) positive antigen test for SARS-CoV-2

(7d) COVID-19 exposure within the 4 weeks prior to the onset of symptoms

 

Laboratory markers of inflammation include:

(1) elevated C-reactive protein

(2) elevated ESR

(3) elevated fibrinogen

(4) elevated serum procalcitonin

(5) elevated serum ferritin

(6) elevated serum LDH

(7) elevated interleukin-6

(8) neutrophilia

 

Additional laboratory markers:

(1) positive for D-dimer

(2) low serum albumin

(3) lymphopenia

 

Some patients may fulfill full or partial criteria for Kawasaki disease.
