Description

The diagnosis of myocarditis or mediastinitis must meet one of the following criteria.


 

Criteria 1: organism isolated from culture of pericardial tissue or fluid obtained by needle aspirate or during surgery

 

Criteria 2 - all of the following:

(1) patient age > 12 months

(2) 2 or more of the following symptoms

(2a) fever (> 38° C)

(2b) chest pain

(2c) paradoxical pulse

(2d) increased heart size

(3) no other recognized cause for the symptoms

(4) one or more of the following:

(4a) abnormal ECG consistent with myocarditis or pericarditis

(4b) positive antigen test on blood

(4c) histologic examination of heart tissue shows evidence of myocarditis or pericarditis

(4d) fourfold rise in type-specific antibody with or without isolation of virus from pharynx or feces

(4e) pericardial effusion identified on echocardiogram, CT scan, MRI, angiography or other radiographic evidence of infection

 

Criteria 3 - all of the following:

(1) patient age <= 12 months

(2) 2 or more of the following symptoms:

(2a) fever (> 38° C) or hypothermia (< 37°C)

(2b) apnea

(2c) bradycardia

(2d) paradoxical pulse

(2e) increased heart size

(3) no other recognized cause for the symptoms

(4) one or more of the following:

(4a) abnormal ECG consistent with myocarditis or pericarditis

(4b) positive antigen test on blood

(4c) histologic examination of heart tissue shows evidence of myocarditis or pericarditis

(4d) fourfold rise in type-specific antibody with or without isolation of virus from pharynx or feces

(4e) pericardial effusion identified on echocardiogram, CT scan, MRI, angiography or other radiographic evidence of infection
