Description

The revised classification system for HIV infection published by the Centers for Disease Control (CDC) with implementation on January 1, 1993 incorporates both the CD4-positive T cell lymphocyte count and clinical findings.


 

Patient populations: adolescents >= 13 years of age and adults

 

CD4-positive Lymphocyte Count

 

CD4-positive T Cell Lymphocyte Categories

(1) >= 500 per µL

(2) 200 - 499 per µL

(3) < 200 per µL, or < 14%

 

where

• the CD4 lymphocyte count is the lowest recorded, not the most recent value

• CD4-positive lymphocyte counts < 200 per µL is an AIDS defining condition

 

Clinical Categories

 

Category A: has not had any of the disorders seen in Category B or C

(1) asymptomatic HIV-infection

(2) acute HIV infection

(3) persistent generalized lymphadenopathy

 

Category B: symptomatic HIV infection not classified as either Category A or C; some of the conditions included with Category B:

(1) bacillary angiomatosis

(2) oropharyngeal Candidiasis (thrush)

(3) vulvovaginal Candidiasis that is frequent, persistent or poorly responsive to therapy

(4) cervical dysplasia or carcinoma in-situ

(5) chronic constitutional symptoms such as fever or diarrhea for more than 1 month

(6) oral hairy leukoplakia

(7) herpes zoster with 2 or more episodes, and/or with involvement of 2 or more dermatomes

(8) idiopathic thrombocytopenic purpura

(9) Listeriosis

(10) pelvic inflammatory disease, particularly if complicated by tubo-ovarian abscess

(11) peripheral neuropathy

 

Category C: symptomatic HIV infection with one or more AIDS-indicator conditions:

(1) Kaposi's sarcoma

(2) Pneumocystis carinii pneumonia

(3) malignant lymphoma, including Burkitt's lymphoma, immunoblastic, or primary lymphoma of the brain

(4) Candidiasis of esophagus, bronchus, trachea or lungs

(5) invasive cervical carcinoma

(6) disseminated or extrapulmonary coccidioidomycosis

(7) disseminated or extrapulmonary histoplasmosis

(8) extrapulmonary cryptococcosis

(9) chronic cryptosporidiosis (> 1 month duration)

(10) chronic isosporiasis (> 1 month duration)

(11) HIV-related encephalitis

(12) cytomegalovirus infection other than liver, spleen or lymph nodes

(13) CMV retinitis

(14) Herpes simplex with chronic ulcers (> 1 month duration), bronchitis, pneumonitis, or esophagitis

(15) Mycobacterium tuberculosis, any site

(16) disseminated or extrapulmonary atypical mycobacteria infection

(17) recurrent pneumonia (2 or more episodes within a 1 year period)

(18) progressive multifocal leukoencephalopathy

(19) recurrent Salmonella septicemia

(20) toxoplasmosis of brain

(21) wasting syndrome due to HIV

 

Interpretation

 

CDC classification =

= (CD4 category) & (clinical category)

 

Patients reported as having AIDS - presence of laboratory confirmed HIV-infection and one or both of the following:

(1) CD4-positive lymphocyte count < 200 per µL and/or < 14% (A3, B3, C3), and/or

(2) one or more clinical AIDS indicators present (C1, C2, C3)
